Our History
Transitions Nursing and Rehabilitation Center was established in 1967 with a desire to set a standard of excellence in skilled nursing services and long-term care.
Since that time Transitions Nursing and Rehabilitation Center has continued to meet the needs of the community. We now offer private Medicare Suites and continue to be a leader in providing quality skilled nursing & rehabilitation services.
Because of our diverse services, we care for patients with a variety of skilled medical needs, including complex medical or respiratory illnesses, traumatic brain injuries, stroke recovery, post-surgical care, and orthopedic rehabilitation. Our comprehensive services focus on our patient’s physical, mental and emotional needs.

General Information
Transitions Nursing and Rehabilitation Center is a 55-bed skilled nursing facility, certified by Medicare and Medicaid. We are a member of the Illinois Healthcare Association. Our RN ratio is well above the local and national average when compared to other long term care facilities.
Our nursing and rehabilitation services benefit patients who may no longer need treatment in a hospital setting, but require continued medical intervention or rehabilitation on a short-term basis, before returning home.
Long term care is also available for those who are suffering from a prolonged acute illness or chronic disease, or for those who simply can no longer live alone at home.
Our professional staff members – from our caregiving team to our friendly support staff – provide a compassionate and supportive environment for healing and recovery. Our caregivers are pre-screened prior to employment, and all are local people – neighbors and friends with hometown values - who are invested in the commitment to excellence for which our facility is known. We are fortunate that many of our staff members have been with us for several decades, providing longevity and experience unique to our profession.
Patients and their families are encouraged to help us develop a plan of care that meets the patient’s physical, social and emotional needs. Whether your stay is short-term or long-term, we work together to help you settle in and feel at home with us.
For residents who plan to return home, we begin discharge planning right away to set realistic expectations and goals, and to coordinate care to ensure a safe return home. As discharge approaches, we may coordinate services through our partners in care to help you in the transition, and to provide continuity of care.
Transitions is a long-term care facility owned or managed by HI Care Management, Inc. located in Springfield, Illinois. The principals of HI Care Management have extensive experience in long-term care, having served in key roles with the Illinois Department of Public Health. They continue work at the forefront of the industry, serving in leadership roles in state and national healthcare organizations to help foster reform and improve the standards of resident care.

Location
Conveniently located in Rock Falls, just minutes from CGH Hospital in Sterling and KSB Hospital in Dixon, with nearby access to I-88 and I-80, we are within walking distance from the Hennepin Canal. We primarily serve the residents of Northwestern Illinois. However, it is not uncommon for us to receive referrals from across the nation.

Partners In Care
Our partners in care include a wide variety of medical professionals, including our Medical Director, attending physicians, physical, occupational and speech therapists, support staff, wound care nurse, licensed dietitian consultant, and licensed social worker consultant. We also contract with professional lab services, pharmacy services, x-ray providers, and a podiatrist to provide ancillary services to our residents. We also work with area home health agencies and other community-based services to arrange services for residents who are able to return home.
