Rehabilitation Services

The focus of rehabilitation is to help residents achieve their maximum level of independence. Our professional rehabilitation team consists of licensed physical, occupational and speech therapists and certified therapy assistants. Our programs have been very successful in helping our residents regain independence and improve their quality of life. In fact, many of our residents are able to return home after completing their rehabilitation.

Physicians assist our team to develop treatment plans for patients who have experienced an injury or illness, such as stroke, amputation, orthopedic impairment, neurological disorder, and other general medical problems. Unlike many hospital therapy departments, we are able to work with patients who may not meet “acute rehabilitation” criteria. This provides an alternative to residents who may find it “too soon” to return home following a hospitalization.

All residents are screened by a licensed therapist upon admission and quarterly thereafter. Following an evaluation of a patient’s functional abilities, an individualized rehabilitation program is developed. The treatment program will address areas related to the patient’s mobility, performance of activities of daily living, communication skills and swallowing function. Knowledge of the patient’s prior lifestyle, interests and hobbies will be used to develop a therapy regimen that is interesting and enjoyable.

A description of our basic therapy services may be helpful:

Physical Therapy – increases strength and flexibility of joints and muscles to resume proper function. It focuses on a patient’s coordination and body mechanics, pain management and confidence.

Occupational Therapy – works closely with our nursing staff to assist the patient and families on achieving independence with activities of daily living, with a focus on self-care skills, home management skills, and improvement in thinking, memory and decision-making skills.

Speech Therapy – improves communication impairments and speaking abilities, and assists in regaining independent eating and swallowing skills.

We also offer advanced modalities, such as ultrasound, electrical stimulation, and diathermy, which serve as effective treatments for pain management, muscle atrophy, circulation problems, stiffness and contractures. These specialized treatment methods set our therapy department apart from the traditional services typically found in long term care.

Management oversight and continuing education for our therapy staff is provided by Rehab Care Group, (www.rehabcare.com) a nationally recognized rehabilitation provider.

Skilled Nursing Services

Our 24-hour skilled nursing services are directed by an R.N. with expertise in geriatric nursing and long-term care. Our nursing team consists of nurse managers, RN’s, LPN’s, and certified nurse assistants. Transitions Nursing and Rehabilitation Center is extremely proud of our high RN staff average. As a skilled nursing facility, we are able to provide a wide range of services, including IV therapies, enteral nutrition, wound treatments and prevention, and end-of life services. Whether providing daily assistance or complex medical care, our nursing team works together to follow the plan of care that has been developed by the interdisciplinary team and attending physician.

We offer a state of the art wound care, Celleration’s Mist Therapy. Mist Therapy using acoustic pressure to heal wounds. Celleration ® Inc. , a Minnesota based medical device company, has produced an impressive and growing body of clinical research with an exciting new wound healing device, the MIST Therapy ® System. MIST Therapy is a painless non-contact ultrasound device that is easy to use with a short treatment time. MIST Therapy promotes wound healing through cellular stimulation while removing bacteria from the wound allowing clinicians to heal wounds faster.

Hospice/Palliative Care

Hospice services are available for residents facing terminal illness or end-of-life decisions. This service includes providing palliative care (comfort care) with the support of a hospice agency and facility staff working together to provide the resident and family with resources, counseling and care through this difficult time. Palliative care is also available without enlisting the services of a hospice agency.

Respite Services

Respite services, or short-term care, is available to individuals needing caregiving support on a very temporary basis, such as a day, a week or a month. Respite care might be appropriate when there is a sudden loss of an in-home caregiver, such as death of a spouse, or when a break from daily caregiving responsibilities is needed. Some caregivers use respite services when they wish to take a vacation, or just need a few days to tend to personal business. In some cases, hospice services may provide costs of respite care for patients enrolled in their program.

Nutritional Services

We understand mealtimes are important to maintaining good health and that good nutrition begins with a healthy desire to eat. We also understand the desire to eat can be diminished when choices are few, and residents feel forced to eat what is served to them. That is why at lunch, our residents are able to eat a restaurant style meal. Our staff, and managers, will assist each resident however necessary. Whether to help encourage appropriate menu selections, or to get settled at their table, our residents receive the assistance they need to ensure a pleasant dining experience.

Our nutritional services are overseen by a Registered Dietitian. Our on-site dietary supervisor oversees preparation of the food and advises staff of individual dietary concerns. We are able to meet the needs of patients on special diets, whether for weight control, diabetic or renal management, supplemental nutrition, or other diet as may be ordered by a physician. Daily snacks are available, and daily menus – for breakfast, lunch and dinner – are posted throughout the facility.

Activity Program

Our Activity program offers opportunities for socialization, and one-on-one programming to meet the individual tastes of our residents. Families are invited and encouraged to participate, and volunteers are welcome.

Discharge Planning

Discharge planning and access to community services are available through our Social Services department. Our Social Service staff will help residents make the transition from their home to ours, and are available to help residents and their families with any issues that may arise during their stay here.

Environmental Services

Housekeeping, laundry and maintenance services are also provided as part of the daily rate. We strive to keep our facility clean, comfortable, and in good repair, making our surroundings as home-like as possible.

Pharmaceutical Services

The facility contracts with a pharmacy which provides medications, medication reviews, and consulting services to the facility. Under the new Medicare D program, the facility is prepared to offer assistance in helping residents review their plans for compatibility of their medication regimen and the formulary their plan offers. Medications are ordered by the facility as prescribed by the physician. It is suggested that residents utilize the services of the facility’s pharmacy provider, which will provide optimum service and 24-hour availability of medications should an emergency arise.

Pastoral Care

Local clergy make regular visits to our facility, and participate in planned religious services. Scheduled religious services will appear on our activity calendar, and may vary from time to time. Some activities are led by volunteers from local churches. Our social service staff will be pleased to assist our residents in notifying their pastor that they are residing at our facility. It is our goal to ensure that each resident’s spiritual needs are met, whatever their beliefs. Please feel free to discuss your religious preferences with our social services staff upon admission.

Other Services

In an effort to meet the individual needs of our residents, our social services staff will assist residents in arranging ancillary services from select providers. This may include dental or eye care, or podiatry and audiology services. Charges for services rendered by these providers are billed independently from the facility charges, and may require an authorization form prior to services being rendered. Some providers will see residents on-site, while other may require transport to and from the appointment. Please discuss your individual needs with our social services staff so appropriate arrangements can be made.
